Service Notice

1. We serve all patients, and no one is denied access to services due to their inability to pay.
2. Discounted/sliding fee schedules for essential services are offered based on family size and income.
3. We do not deny services to patients based on a person’s race, color, sex, age, national origin, disability, religion, gender identity, or sexual orientation.
4. We accept health insurance, including commercial insurance, Medicaid, Medicare, and the Chil-dren’s Health Insurance Program (CHIP).
